At Animal Health Services we realize that you know your pet best. By observing behaviors related to itching and scratching at home, you can help us determine the severity and cause of this irritating symptom.

The forms below will help you keep track of your pet’s symptoms and also allow for us to see what goes on outside of the exam room. Print, fill out & bring these forms in during your pet’s physical exam appointment for your doctor to look over the notes of your pet’s condition.

Evaluation Form – A thorough history can help us find the source of your dog’s itching more quickly. Please answer the following questions to help guide the diagnostic process.

Daily Itch Report Card – Keep track of how itchy your dog is for the next 30 days. Measure severity of itch on a scale of 1-10, 1 being mild and 10 being the most severe. Bring this report card back at your next visit.
