AHS Pet Fit Club fees are due at the first attended meeting.

You are also signing that you understand the fee is non-refundable. Meetings are encouraged but not mandatory. Fee is the same whether pet attends or is absent for the next consecutive 12 meetings from the signing of this form. In the event a meeting time or date must be changed, Animal Health Services reserves the right to change meeting times or venues at their discretion. Any extenuating circumstances such as illness, injury or death that prevents a pet from attending a meeting will be dealt with on a per event case by the management of Animal Health Services.
